How to Access the Dr. Brent Ohl Strong Pancreatic Cancer Support and Awareness Fund
The Dr. Brent Ohl Pancreatic Cancer Support and Awareness fund wants to support you in your fight against pancreatic cancer. We can provide you with $500 in Visa gift cards, which can be used as you wish. This is a symbol of our support and to show you, you are not alone as well as others care about you as you battle this cancer.
What requirements are needed to be eligible for our support?
1. Have a pancreatic cancer diagnosis and provide documentation
2. Live within 75 miles of the UnityPoint Rock Island Hospital.
There are designated contacts available at the UnityPoint Cancer Center, The Genesis Cancer Center and the University of Iowa Cancer Center. If you are a patient at one of these 3 locations, tell your cancer center contact, you want to have access to our support fund. Their designated contact will know how to get you our support packet. They will either give you the packet in person or it will be sent to you from the Trinity Health Foundation.
Where to access our packets:
1. If you are a patient at the UnityPoint Cancer Center, ask the Cancer Center Social worker to access this packet for you.
2. If you are a patient at the Genesis Cancer Center, ask the cancer center social worker or nurse navigator to access this packet for you.
3. If you are a patient at the University of Iowa Cancer Center, ask the social worker to access this packet for you.
4. If you are a patient at another cancer center, but you still meet the two above requirements, please have your providers office contact the Trinity Health Foundation at 309-779-7610.
If you are eligible based on our two requirements, and are having difficulty accessing our packet and have not received our packet, please request your providers office contact the Trinity Health Foundation at 309-779-7610.
One special reminder, once you receive our support, we want to honor you on our shirts and sweatshirts.
There is a card included in each packet, giving approval to be included on our shirts and sweatshirts and how you want your name to appear. Please complete that card and return it in the self-addressed envelope.
